For a first visit, the €26 Cathedral + Terraces ticket is the one to buy — it's the cheapest option that includes the rooftop, the part of the Duomo you can't get anywhere else. Skip the €12 cathedral-only ticket unless you're certain you won't go up.

In high season, yes — popular time slots sell out days ahead, and about 20 of the 120 recent reviews we analysed advise booking early. Note that no ticket skips the compulsory security check; a 9–10am weekday slot is what actually avoids the queue.
The rooftop is the most-praised part of the visit (76 of 120 recent reviews mention it). The stairs are around 250 steps; the lift goes up only, to a lower terrace — everyone climbs the last ~90 steps and walks down, so take the stairs if you're reasonably fit and the lift in heat or with knee trouble.
Shoulders and knees covered for everyone inside the cathedral. Cover-ups are sold at the ticket machines for about €3; the rule isn't enforced on the rooftop.
Plan 2–3 hours for the cathedral and terraces. The full pass with museum and archaeological area fills most of a half day — and the museum is closed on Wednesdays.
